         5.4.2.4 Inibidores de protease

Esquemas contendo inibidores da protease têm sido avaliados por estudos com desfechos clínicos, como morte e progressão para Aids, evidenciando eficácia em pacientes com infecção avançada pelo HIV, em infecção em estágio sintomático, infecção assintomática associada à imunossupressão grave (contagens de CD4 inferiores a 200 células/mm3) ou resgate terapêutico2. A associação de ritonavir como adjuvante farmacológico em esquemas contendo inibidores de protease aumenta as concentrações plasmáticas destes, por interação farmacocinética. A associação também reduz a resistência a lamivudina36. inibidores de protease estão associados a hiperglicemia e lipodistrofia, devendo ser usados com precaução em diabéticos. Como são geralmente metabolizados pelo complexo citocromo P450, estão envolvidos em um grande número de interações medicamentosas1. As evidências para subsidiar decisões em caso de falha dos esquemas de primeira linha são limitadas, mas os desfechos encontrados em estudos com esquemas de segunda linha incluindo inibidores de protease associados a ritonavir são em geral favoráveis37.

Lopinavir + ritonavir (LPV/r), associação em dose fixa, constitui opção de primeira escolha quando inibidores da protease fazem parte do esquema inicial de tratamento38, inclusive crianças, podendo também ser usado por pacientes que já utilizaram outros antirretrovirais39 (ver monografia, página 815).

Ritonavir (RTV ou r) não é usado como antirretroviral por si mesmo, sendo recomendado em associação com outro IP, como um adjuvante farmacológico1 (ver monografia, página 931).

Saquinavir (SQV), administrado com ritonavir, zidovudina e lamivudina, é bem tolerado em grávidas e neonatos. Nas mães, houve diminuição da carga viral no momento do parto. Nos recém-nascidos, ocorreram anemia, neutropenia e hiperbilirrubinemia40. Saquinavir associado a ritonavir em baixa dose pode ser considerado opção em pacientes virgens de tratamento ou naqueles que não usaram IP41 (ver monografia, página 936).

Atazanavir (ATV) costuma ser associado a ritonavir, em esquema de dose única diária, e segundo revisão sistemática42, esta associação resulta em menores concentrações plasmáticas de lipídios do que outros inibidores de protease associados a ritonavir. Pode ser administrado nas doses usuais em pacientes com problemas renais1. Regimes com atazanavir/ritonavir levam à seleção de mutações que conferem resistência cruzada com outros IP. É alternativa para o tratamento inicial quando regimes baseados em inibidores da protease são preferíveis aos que empregam ITRNN43 (ver monografia, página 973).
